All registered patients at Minnesota Personalized Medicine can purchase the following services and products.
All appointments require a valid credit card on file.
Payment is due at the time of service and will be charged to the credit card on file.

New Patient Appointments
New Patient appointments are based on the physician's hourly rate.
The appointment cost includes face-to-face time with your physician (typically 2-5 hours, averaging 3-1/2 hours) plus additional time necessary for medical record review and charting preparation (ranging from 0-2 hours or potentially more depending on complexity). It also includes assessments and a co-created Action Plan.
The New Patient appointment cost is typically $980-$3,850 depending on the physician and time involved.
New Patient appointments can be scheduled as an in-person appointment in our clinic or a telemedicine appointment.
Follow-Up Appointments
Follow-Up appointments are based on the physician's hourly rate.
While the appointments can range from 30 minutes to two hours, they are typically 1-1/2 hours long. They are scheduled in accordance with complexity and time required.
The Follow-Up appointment cost is typically $245-$1,100 depending on the physician and time involved.
The number of Follow-Up appointments will vary from patient to patient. A patient should plan to return for a total of two Follow-Up appointments on average. However, more complex cases may require more.
Follow-Up appointments can be scheduled as an in-person appointment at our clinic; as a telemedicine appointment; or as a phone consultation.
Telemedicine Appointments
Telemedicine appointments are based on the physician's hourly rate. They are available upon request and require appropriate scheduling.

Overall
Test kits and supplement costs are not included in the appointment cost.
Payment is due at the time of service.
All appointments require a valid credit card on file.
We strive to be flexible and to be of the greatest service to you. Therefore, all New Patient appointments are blocked for three hours with flexibility to extend beyond, if necessary.
For this reason, all new patients who fail to cancel within seven, full business days will be charged for a 1-1/2 hour appointment. This fee is non-refundable.
Authorizations, Letters of Medical Necessity, etc. – $50+ based on complexity & physician time required
Patient Portal Messages – Complimentary when answered by support staff; Complimentary when asking for clarification of an Action Plan; $50+ when answers require physicians’ time, expertise and/or charting
Phone Messages – Complimentary when answered by support staff; Complimentary when asking for clarification of an Action Plan; $50+ when answers require physicians’ time, expertise and/or charting
The fee for the ANS test is $250.
This fee includes a 15-minute appointment with your physician to review your results. This appointment must be scheduled separate from the ANS test.
In addition to the cost of a lab test, additional fees may apply:
Blood Draw/Phlebotomist Services – $75
Lab Order Sent to Other Labs – $35 per order
Test Kit Administrative Fee – $35 per kit
Physician Review of Medical Records – Based on physician's hourly rate
Transfer of Medical Records – Complimentary to other physicians & insurance companies; Digital copies are complimentary to patient via patient portal; Hard copies sent to patient per USPS mailing rates (Applies to Action Plans, test results, etc.)
Complex Prescription Management – $25
Complex Prescription Pre-Authorizations – $50
Hand-Carry Prescriptions – $25 (Hand-written & signed by physician)
Pre-Authorization Prescription Form Completion – $25+ depending on time required
IV Infusion Therapy – Ranging from $60-855
Platelet-Rich Plasma (PRP) Treatment – $875 (Single treatment)
In-Clinic Supplement Sales - At cost (Approx. 15-25 % off retail pricing)
