All appointments require a valid credit card on file.
Payment is due at the time of service and will be charged to the credit card on file.

New Patient Appointments
New Patient appointments are based on the physician's hourly rate.
The appointment cost includes face-to-face time with your physician (typically 2-1/2 to 4 hours) plus additional time necessary for medical record review and charting preparation (ranging from 0 to 2 hours or potentially more depending on complexity). They also include assessments and a co-created action plan.
Test kits and supplement costs, however, are not included in the appointment cost.
Follow-Up appointments are based on the physician's hourly rate.
They range from 30 minutes to 2 hours, but are typically 1-1/2 hours long. They are scheduled in accordance with complexity and time required.
The number of Follow-Up appointments will vary from patient to patient. A patient should plan to return for a total of two Follow-Up appointments on average. However, more complex cases may require more.
Telemedicine appointments are based on the physician's hourly rate. They are available upon request and require appropriate scheduling.
Phone consultations are based on the physician's hourly rate. They are available upon request and require appropriate scheduling.
All new patients who fail to cancel within four, full business days will be charged for a 1-1/2 hour appointment.
This fee is non-refundable.
Authorizations, Letters of Medical Necessity, etc. – $50+ based on complexity & physician time required
Patient Portal Messages – Complimentary when answered by support staff; $50+ when answers require physicians’ time, expertise & charting
Phone Messages – Complimentary when answered by support staff; $50+ when answers require physicians’ time, expertise & charting
Blood Draw/Phlebotomist Services – $75
Genova Diagnostics Test Kits – $0-400 (Depending on insurance reimbursement; Applies to GI Effects & NutrEval tests)
Lab Order Sent to Other Labs – $35 per order
Test Kit Administrative Fee – $35 per kit
Physician Review of Medical Records – Based on physician's hourly rate
Transfer of Medical Records – Complimentary to other physicians & insurance companies; Digital copies are complimentary to patient via patient portal; Hard copies sent to patient per USPS mailing rates (Applies to action plans, test results, etc.)
Complex Prescription Management – $25
Complex Prescription Pre-Authorizations – $50
Hand-Carry Prescriptions – $25 (Hand-written & signed by physician)
IV Infusion Therapies – Ranging from $60-855
PRP Treatment – $875 (Single treatment)
In-clinic supplement sales are at cost (Approx. 15-25 % off retail pricing)
